The online form for new web hosting service is used for creating new hosted accounts and associated web services with ITS.  The form can be found going to https://servicedesk.its.uq.edu.au, click on Service Catalogue, and then clicking on 'Request Service' in the 'New Web Hosting Service' box.

The pink fields, are all the mandatory fields.

Filling out the form

Section I.

  • The 'Department' field is the department of the person for who the request is being made, this may be you, or the person who has asked you to make the request.
  • The 'Requested for' section should be the person for who the request is being made, this may be you, or the person who has asked you to make the request. 
  • The 'Requested by' section is the person making the request.  This will generally be you.
  • Use the 'Contact Details' section to specify how you wish to be contacted about the request.  Please specify alternate contact details if the defaults are not correct.

Section II.

  • The 'Justification' field should be filled out with the reason for the new hosting services.
  • 'Management Approver' should be filled out with the name of the manager who is approving the new hosting services.  The request will be sent to them for approval.
  • 'Financial Approver' should be filled out with the name of the financial officer who is authorising the cost of new hosting services.  The request will be sent to them for approval.
  • 'Date required' is an optional field to specify when you would like the new service to be created.  If this is left blank, it will be assumed that the new service to be created as soon as possible.

Section III (Option 1).

This section only needs to be filled out if you are requesting an Altera service.  Skip this section if you only want to create a hosted server account.

  • The 'Top Level Domain' field should be filled out with the top level prefix of the UQ website address you want to use, ie use 'its' for 'www.its.uq.edu.au'.
  • 'Host Master Approved?' refers to if you have been granted authorisation from the ITS Hostmaster.  To get authorisation, you will need to email hostmaster@uq.edu.au or alternatively contact the ITS Service Desk.
  • The 'Web Alias' field refers to the end of the URL in which this new UQ website address should use, ie use 'its' for 'uq.edu.au/its'.
  • 'DB Name - Altera Only" should be filled out with the name of the database you plan on using for this hosting service.  This only applies if you are requesting an Altera service.  The second field is for your next best preference incase the first database name is already taken.

Section III (Option 2).

For this section you are given a first and second preference.  You will need to fill in both incase your first preference is not available.

  • The 'Acc Name 8 Chars Max:' should be filled out with the preferred account name of the hosted server account.  The username may only contain up to 8 letters or numbers.
  • 'Email - 26 Chars Max:'  should be filled out with the preferred email address for the hosted server account.  Each hosted server account is granted an email account.  The email address may only contain a maximum of 26 letters or numbers.
  • 'User Name - 8 Chars Max:'  should be filled out with the preferred username of the hosted server account.  The username may only contain up to 8 letters or numbers.

​Section IV.

Only fill in the section that relates to the type of account being requested.  For example, if it is a hosted server account for a 'Research Affiliate' fill on 'Section IV Option 2' only.

  • The 'Altera Virtual Hosting incl 200MB/PHP/My SQL' checkbox should be ticked if you want an Altera Virtual Hosting Service.  Information on the features of a Altera virtual hosting service can be found here.
  • 'With OCMP Hosting / PHP  (Please Indicate)' checkbox should be ticked if required.  You will then need to select 'Python' from the drop down menu.
  • 'Additional Web Space (Qty 2 = 200MB)' should be filled in if any extra web space is needed.  Note each number refers to 100MB blocks of additional diskspace required.  Depending on the service, you will start with 100MB or 200MB initially.  Any extra blocks will be accumulated against the initial amount.

Section V.

  • Fill out the unit to be billed field with the name of the organistational unit that is paying for your hosting account
  • The next 7 fields are for UniFi code information, if you do not know this information, please consult with the person you specified as the financial approver in Section II.

Once you have completed the form.  Click 'Save' at the top of the page.

At this point the form is only saved, not submitted.  This allows you to come back and make changes before submitting the form.

When you are happy that the form is correct, click 'Submit Request', from the actions menu in the top left.

Approvals

After submission, the request will be Awaiting Management Approval. At this point an email will be sent to the manager you specified when filling out the form.

After management approval, if the manager requests is, the request will be Awaiting Financial Approval. At this point an email will be sent to the financial officer your specified when filling out the form.

Processing

After financial approval, or if financial approval is not required, the request will be in the status of Provisioning, this means the request is with the Client Services team for processing.

Once the Client Services team has processed your cancellation request, you will receive an email informing you that the request is now resolved.
